The program will last a year and will offer over 60 families of dementia patients home visits. The intervention for each family will last four months, with about 20 families being served per quarter. The program is staffed by a team that includes a psychologist, a social worker, a physical therapist, a case manager, a nurse, and a psychiatrist. Apart from the services provided by each specialist, the staff is available via telephone for further support to the caregivers. The program’s goal is to prolong the presence of dementia patients in their home, thus avoiding institutionalization.
New Program Creation: Care at Home Program for Dementia Patients
The Greek Association of Alzheimer’s Disease and Related Disorders of Chalkida created, through a grant of the TIMA Charitable Foundation, a care at home program for patients with dementia who reside in the greater area of Chalkida.
