This Care at Home Program will target elders recently diagnosed with stage one, two and three of dementia, as well as their caregivers. The purpose of this Program is to prolong the presence of these individuals at home. During this continuation phase of the program, a physiotherapist and a nurse will be added to the existing team, complementing the social worker, the psychologist, and the physical education expert that were already present from the pilot phase of the program. More specifically, the services offered are: mental health strengthening exercises, psychological support to both the elder and the caregiver, and physical strengthening exercises along with specialized advice on nutritional regime. Emphasis will be placed on training of the caregivers in order to improve the care and mobility of the patients. Frodizo will continue offering training seminars for caregivers and will also organize monthly group meetings for caregivers in order to share their concerns and support each other. During the continuation phase of the program, the beneficiaries will be approximately 45 elderly residents of Patras.
